Eastwick Family Service is a registered Social Services provider in the State of Pennsylvania. We provide services for individuals with various disabilities. Our Dynamic staffs are here to accommodate you and make the referral process as simple as possible. Eastwick presently provides services in the following counties: Delaware county, Philadelphia County, Montgomery county, Bucks County, and Chester county.
Eastwick provides the following services; Personal Assistance Services, Respite in Home service, Companion Services, and Home and community Habilitation Services. We are registered to provide services for an individual under the office of Long-term living, and the office of Developmental Program. Please see below for the kind of Waiver we are presently qualified to provide services under the office of long term living: Attendant Care Waiver Independent Care Waiver, Obra Waiver, Commcare waiver, and Aging waiver.
We provide Services under the office of Developmental Program for adults with intellectual disabilities. Within this department, we provide the following services: Companion Services, Home and Community Habilitation services. Respite in home Services. Individuals who are consolidated Waiver or PFDS waiver can receive the services as indicated above. To qualify for Services, an individual must be enrolled in the Waiver program. County offices and support coordination organizations can assist an individual in getting into the waiver program. Please contact your County office or Service Coordinator Organizations regarding enrollment.
