Office Of Long Term Disability

Eastwick Family Services is a provider organization that provides services for individuals with various disabilities. Eastwick is registered with Pennsylvania Office of long-term living to provide services for individuals 65 years or older.

Services are also offered for other waivers. Please see below if you’re qualified for services under the listed waivers.

Aging Waiver

What services are available?

Home health and personal care services, home support, attendant care, respite care, adult day care, transportation, home modifications, specialized medical equipment and supplies, counseling, extended state plan physician services, home-delivered meals, personal emergency response, and companions. Care management and service coordination are provided by the Area Agency on Aging:
Who is eligible?

You must be a resident of Pennsylvania, at least 60 years of age, and meet all of the following criteria:

  • Be clinically and financially eligible for MA services
  • Can be served in the community by available health and social services;
  • Meet the financial requirements, as determined by your local county assistance Office.
  • Agree to the requirements and responsibilities of the program.

How does the program work?

Each Aging Waiver participant will have the support of their Area Agency on Aging. The agency’s staff will regularly assess your needs; help develop and maintain an appropriate plan of care; assist in selecting Medical Assistance and program approved services and product providers, and monitor the quality of the services provided. Contact your local Area Agency on Aging for more information regarding the Aging Waiver.

Commcare Waiver

To be eligible for the COMMCARE Waiver you must:

  • Be 21 years of age or older.
  • Have a medically determinable diagnosis of traumatic brain injury and require a Nursing Facility (NF) level of care.
  • Meet the financial requirements as determined by your local County Assistance Office.

Services We provide

  • Community Integration.
  • Home Health.
  • Personal Assistance Services.
  • Respite.

Attendant Care Waiver

To be eligible for either the Attendant Care Act 150 Program or the Attendant Care Waiver, individuals must be at least 18 years of age but less than 60 years of age and be mentally alert and capable of:

  • Selecting, supervising and, if needed, firing an attendant
  • Managing one’s own financial affairs
  • Managing one’s own legal affairs

Eastwick Provide Personal Assistant services, Companion Services, and Respite Services for individual under the Attendant care waiver

  • Experience any medically determinable physical impairment that can be expected to last for a continuous period of not less than 12 months or that may result in death.
  • Due to physical impairment(s), require assistance to complete functions of daily living, self-care, and mobility.
  • Be a citizen of the United States or immigrant lawfully admitted for permanent residence.
  • Be a resident of Pennsylvania

Additional requirements to participate in the Attendant Care Medicaid Waiver:

  • Individuals must require nursing home care facility level of care for a period of 12 months or longer.
  • Possess income and resources within limits established in the Medicaid Waiver for Attendant Care Services.

Additional requirements to participate in the Attendant Care Act 150 Program:

  • While most consumers do not pay into the Attendant Care Act 150 Program, individuals with higher income may be assessed a minimal co-payment. This co-payment is based on the consumer’s income and will never be more than the total cost of services.

Independence Waiver

If you are an adult with a severe physical disability, the Independence Waiver may be able to help you live or remain in the community and remain as independent as possible.

To be eligible for the Independence Waiver you must:

  • Be a Pennsylvania resident
  • Be 18-60 – Individuals that turn 60 while in the waiver will be able to continue to receive services through the Independence Waiver.
  • Individuals who are physically disabled (but not individuals with an intellectual disability or have a major mental disorder as a primary diagnosis, or who are ventilator dependent), who reside in a Nursing Facility (NF) or the community but who have been assessed to require services at the level of nursing facility level of care.
  • In addition, the disability must result in substantial functional limitations in three or more of the following major life activities:
  • Self-care, understanding, and use of language, learning, mobility, self-direction and/or capacity for independent living.
  • Meet the financial requirements as determined by your local County Assistance Office.

  • Accessibility Adaptations, Equipment, Technology and Medical Supplies
